Vancomycin case studies

Case 1

A 45 year old female is admitted to general med with MRSA. Admission data:
• SCr = 0.9 mg/dl
• Wt = 165#
• Ht = 5' 9"

Enter the patient data into Kinetics and determine a Vancomycin treatment plan.

• Remember to keep the trough above 5 mcg/ml.

After 2 days of the above therapy, a trough level is drawn 30 minutes before the start of the 60 minute infusion. The trough level was reported as 15 mcg/ml.

Enter the serum level data into Kinetics and determine a new Vancomycin treatment plan.

Case 2

A 34 year old male is admitted to oncology with FUO. Admission data:
• SCr = 0.9 mg/dl
• Wt = 165#
• Ht = 67"

Enter the patient data into Kinetics and determine a Vancomycin treatment plan.

• Remember to keep the trough above 5 mcg/ml.

After 3 days of the above therapy, a serum level is drawn 60 minutes before the start of a 60 minute infusion. The level was reported as 7.5 mcg/ml.

Enter the serum level data into Kinetics and determine a new Vancomycin treatment plan.
